Vision & Hearing Screening Services: Polk County Health Department

The Polk County Health Department performs vision and hearing screenings, referrals, and follow-up site visitations on children ages 3 to 5 in child care sites in Polk County. These services are provided at Child Care Centers and Family Child Care Homes (FCCH) at the Coalition’s direction. If time permits during visits to child care sites a health education component will be provided.
